This Statement of Work (“Statement of Work” or “SOW”), is made effective as of (DATE) between (“Client”) and Global Risk Management Solutions, LLC, a Delaware limited liability company (“GRMS”) and is being entered into in connection with and incorporates by reference the terms and conditions of the Master Services Agreement between GRMS and Client, (DATE) (“MSA”). Any capitalized term which is not defined herein has the meaning given such term in the MSA.

1. Term The term of this Statement of Work (“SOW”) is to run concurrently with the term of the Master Services Agreement.
2. Services to be Performed and Costs  GRMS will provide supplier risk assessment services for Client’s suppliers based on the risk assessment package(s) detailed in Exhibit A of this SOW.

Costs for GRMS’ risk assessment services will be borne by Client’s suppliers at the time of initial program enrollment and subsequent annual re-enrollment. GRMS will not charge Client for any services or support associated with its supplier risk assessment services.

The annual supplier funded price for this risk assessment program will be ($XXX USD) per year for suppliers in the U.S. and Canada.
